ABSTRACT

Objective:To understand the diagnostic value of tuberculosis (TB) pathogenic detection methods (TPDM) in pathological tissue for TB.Methods:A retrospective study was conducted with 190 pathological specimens from different tissues suspected with TB from Third People′s Hospital of Shenzhen during May 2016 and May 2019. Specimens were divided into four groups according to histomorphology: group one, necrotizing granulomatous inflammation (109 cases); group two, non-necrotic granulomatous inflammation (20 cases); group three, non-granulomatous inflammation (45 cases); group four, non-tuberculous lesions (16 cases). The positive rates of each TPDM among specimens from four groups were compared. The positive rates of all TPDM for specimens from group one were compared. Meanwhile, the influence of antituberculosis treatment course on the TPDM was analyzed. Chi-square test or Fisher′s exact test was used for statistical analysis.Results:The positive rates of Ziehl-Neelsen acid-fast staining among the four groups were 17.4%(19/109), 5.0%(1/20), 4.4%(2/45) and 0(0/16), respectively. The positive rates of Mycobacterium tuberculosis (MTB) complex culture were 32.0%(32/100), 4/19, 4.8%(2/42) and 0(0/16), respectively. The positive rates of Mycobacterium tuberculosis/rifampin resistance real-time quantitative nucleic acid amplification detection system (Xpert MTB/RIF) were 74.3%(81/109), 15.0%(3/20), 13.3%(6/45) and 0(0/16), respectively. The positive rates of fluorescent quantitative polymerase chain reaction (FQ-PCR) were 63.0%(58/92), 0(0/15), 2.6%(1/38) and 0(0/10), respectively. The positive rates of simultaneous amplification and testing (SAT) were 32.4%(24/74), 0(0/10), 0(0/15) and 0(0/10), respectively. The differences of each TPDM among four groups were all statistically significant (all P<0.05). The positive rate of Xpert MTB/RIF in group one specimens was significantly higher than those of acid-fast staining, MTB culture and SAT ( χ2=71.016, 37.162 and 35.679, respectively, all P<0.01), while the difference was not statistically significant when compared with FQ-PCR ( χ2=2.517, P=0.112). The positive rate of combined TPDM (85.3%(93/109)) was significantly higher than Xpert MTB/RIF(74.3%(81/109)) ( χ2=4.100, P=0.043). The positive rates of acid-fast staining group 1A (anti-tuberculosis treatment course was less than one month) and group 1B (anti-tuberculosis treatment course was longer than one month) were 14.3%(7/49) and 20.0% (12/60), respectively ( χ2=0.612, P=0.434); those of MTB culture were 48.9% (22/45) and 18.2% (10/55), respectively ( χ2=10.721, P=0.001); those of Xpert MTB/RIF were 69.4%(34/49) and 78.3%(47/60), respectively ( χ2=1.131, P=0.287); those of FQ-PCR were 55.0%(22/40) and 69.2%(36/52), respectively ( χ2=1.965, P=0.161); those of SAT were 43.3%(13/30) and 25.0%(11/44), respectively ( χ2=2.736, P=0.098). Conclusions:The results of TPDM correlate closely with the typical histomorphological features of tuberculosis. Xpert MTB/RIF possesses significantly higher sensitivity than any other single TPDM, and is not attenuated by early anti-tuberculosis treatment. Combined TPDM could significantly improve the sensitivity of TB pathogenic detection, which is suggested to be applied when the tissue specimen is sufficient.

ABSTRACT

Objective:To study the soil physical and chemical properties, microorganisms, and metabolites in different culture environments of <italic>Gastrodia elata</italic>, so as to provide scientific basis for subsequent cultivation of <italic>G. elata</italic> in multiple environments. Method:The tubersphere soil of <italic>G. elata</italic> cultured in different environments was collected for analyzing the soil nutrients, microbial numbers, and metabolite differences using the agrochemical method, plate-count method, and gas chromatography-time-of-flight mass spectrometry (GC-TOF-MS)-based non-targeted metabonomic approach. Result:The analysis of soil physical and chemical properties revealed the highest soil moisture, pH, available potassium, and available phosphorus in the spinney and the highest electrical conductivity, total nitrogen, alkali-hydrolyzable nitrogen, and organic matter in the pinewood. As demonstrated by the quantitative analysis of soil microorganisms, the cultivable microorganisms were bacteria, actinomycetes, and fungi, with the bacterial population and total microbial biomass in the spinney and the number of fungi and actinomycetes in the barren slope detected to be the largest. The ratio of bacteria to fungi (B/F value) in the pinewood was the highest, while that in the barren slope was the lowest. The results of metabonomic research demonstrated that the compositions and quantities of soil metabolites in the spinney (Z group), pinewood (S group), and barren slope (HD group) varied. Through comparisons between S and Z groups, between HD and Z groups, as well as between HD and S groups by orthogonal partial least squares discriminant analysis (OPLS-DA), 18, 35, and 24 differential metabolites were separately screened out, and the Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment analysis yielded 5, 9, and 13 metabolic pathways. There existed a significant causal relationship of the soil physical and chemical properties and microbial numbers with the metabolites. Conclusion:The soil physical and chemical properties, microbial numbers, and metabolite changes differed significantly in different culture environments of <italic>G. elata</italic>, which were sorted by the suitability in a descending order as follows: spinney > pinewood >barren slope. The soil physical and chemical properties and microbial numbers are the crucial factors driving changes in soil metabolites, suggesting that regulating the soil physical and chemical characteristics and microbial characteristics in the culture environment is an important mechanism for maintaining the <italic>G. elata</italic>-soil-microbial symbiotic system.

ABSTRACT

Objective:To investigate the factors affecting the quality of post-harvest Angelicae Dahuricae Radix (Baizhi) and to provide the corresponding suggestions according to these factors. Method:A series of factors affecting the quality of Baizhi were analyzed comprehensively by visiting several origins, medicinal material markets, pharmacies, supermarkets and storehouses combined with plenty of literature reports. Result:The rot of Baizhi was the most common phenomenon after harvest, and mildew, discoloration and moth could occur during the processes of processing, storage and packaging. So the quality of Baizhi should be controlled strictly through the whole process. The roots of Baizhi should be harvested on sunny days to avoid mining damage and dried as soon as possible to prevent rotting. The drying methods included both traditional lime burial and modern ways such as sulfur fumigation, saline dehydration, sun drying method, and oven drying method. However, the water content should be strictly controlled. Furthermore, the storehouses should be disinfected firstly. The temperature and humidity of the storehouses should be controlled strictly. Some suitable ways could be adopted to prevent moth and discoloration during storage, which included airtight fumigation of aluminum phosphide, cold storage, air conditioning maintenance, antagonism storage with other Chinese medicinal materials, dry ice storage at room temperature, etc. Large quantities of Baizhi were generally packaged in woven bags. Otherwise, kraft paper bags, polyvinyl chloride plastic bags, glass bottles, aluminum foil composite film bags, and other packaging materials had been adopted for the retail. However, the packaging specification should be selected according to the situation. Conclusion:The quality of Baizhi can be directly affected by harvesting, drying, processing, storage and packaging. Scientific and reasonable methods should be adopted to ensure the quality of Baizhi.

ABSTRACT

Objective:According to the GB/T 15000.3-2008, to develop a fucosterol certified reference material based on the project approved by Standardization Administration. Method:Fucosterol was isolated from <italic>Laminaria japonica</italic> dried thallus via 95% ethanol extraction, vacuum concentration, repeated column chromatography separation, recrystallization in petroleum ether-ethyl acetate, and residual solvent removal. Its chemical structure was identified by elemental analysis (EA), infrared spectrum (IR), mass spectrometry (MS), nuclear magnetic resonance (NMR) and X-ray diffraction (XRD). Its homogeneity, stability, and cooperative certification conducted by 8 laboratories were carried out by high performance liquid chromatography with evaporative light scattering detector. Result:For the fucosterol reference material, the certified value of purity was 99.54% with expanded uncertainty of 0.16% in confidence interval of 95%, the stability was good within 24 months storage period at 2-4 ℃, which met the technical requirements of reference material and passed the acceptance organized by Standardization Administration. Conclusion:The national standard materials of fucosterol has been successfully developed, which can be used for the determination of this component, the evaluation of detection methods, and the detection and quality control of related products.

ABSTRACT

Objective:To further improve the department information system, and to establish a multi-parameter critical care medicine database, which can provide data for the analysis and research of big data in critical care medicine, and provide references for other medical institutions to establish relevant databases.Methods:On the premise of fully understanding the needs of clinical and scientific research, based on a Critical Care Medicine Clinical Information System, the department of critical care medicine of the First Medical Center of Chinese PLA General Hospital integrated the patients' case data in hospital information system (HIS), electronic medical records (EMR), monitoring information system (Monitor), laboratory information system (LIS), and radiation information system (RIS), to establish a rudimentary critical care database. On this basis, the related data were analyzed and verified. Further, this database was gradually improved in both its content and structure by referring to Medical Information Mart for Intensive CareⅢ (MIMIC-Ⅲ) database.Results:During the operation of Critical Care Medicine Clinical Information System from September 2017 to February 2020, the database collected diagnosis and treatment data of 2 207 critically ill patients, including data before the patient entering the intensive care unit (ICU) and all data during the ICU, such as demographic data, vital signs, medical treatment, the records of intake and output, sampling time, laboratory examination results, surgical treatment, and a variety of commonly used clinical scoring and diagnosis data. The data in the database were stored in different tables according to different contents, and the tables were connected to each other through the primary key. The data could be analyzed statistically through the information system and has been applied for certain clinical studies, combining clinical practices with scientific studies.Conclusions:The critical care medicine database based on the Critical Care Medicine Clinical Information System can help medical institutions to carry out standardized treatment and clinical research of critically ill patients. With further improvement of the function, the database can be better applied to the data analysis of Chinese critical patients.

ABSTRACT

Objective:Fresh tubers of Gastrodiae Rhizoma were harvested at the right time. A saline water salting and drying technology was developed for obtaining the medicinal materials of Gastrodiae Rhizoma in the place of origin and avoiding rot and mildew. Method:Fresh tubers of Gastrodiae Rhizoma were dug in Yiliang,Yunnan,Dejiang,Guizhou,and Chenggu,Shaanxi,the experiments of natural drying,and saline water salting and drying were carried out in the place of origin and Beijing. After the dirt was removed,the samples were tiled in a container immediately,added with varied proportions of saline water (0.03-0.10 g·mL-1 NaCl in water),hermetically pickled for 6-15 d. after being soaked and rinsed with water,the samples were put in a cool ventilated place or under sunshine to prepare dried medicinal materials of Gastrodiae Rhizoma. We described the appearance characteristics,measured the moisture content,gastrodin and nitrite. And the appearance was observed after storage in a simple warehouse for one year later. Result:Fresh tubers of Gastrodiae Rhizoma from three origins were naturally dried,the surface of gastrodia tubers became black,decayed and moldy,then we could not get dried medicinal materials. The appearance and the content of gastrodins in the medicinal materials of Gastrodiae Rhizoma processed by saline water salting and drying technology met the requirements for Gastrodiae Rhizoma in the Pharmacopoeia of the People's Republic of China 2015 and relevant standards of nitrite in salted food in National Food Safety Standard Determination of Nitrite and Nitrate in Foods,Hygienic Standard for Preserved Vegetables,Green Food-Soybean Paste and Salted Vegetable. Conclusion:The saline water salting and drying technology is developed to make medicinal materials of Gastrodiae Rhizoma quickly from fresh tubers of Gastrodia elata in the place of origin and Beijing. The metamorphism had not been observed after being stored in simple warehouses for one year. This technology can guarantee the quality of Gastrodiae Rhizoma, and provide a new method for the filed processing of Gastrodiae Rhizoma.

ABSTRACT

Objective: To observe the effect of an active fraction of Polyrhachis vicina (AFPV) on systemic lupus erythematosus (SLE) and its possible mechanism based on animal and cell models. Method: Totally 60 SD rats were randomly divided into normal control group, model group, prednisone acetate group (5 mg·kg-1), and high, medium and low-dose AFPV groups (400, 200, 100 mg·kg-1). SLE model was replicated with bovine serum albumin-Freund's complete (incomplete) adjuvant. Arthus reaction was observed to study the effect of AFPV on the diameter of back skin redness in rats with SLE. The expressions of anti-double-stranded DNA (dsDNA) antibody, complements 3 (C3), complement 4 (C4), immunoglobulin M (IgM), interleukin-1 (IL-1), interleukin-6 (IL-6), interleukin-31 (IL-31) and interleukin-33 (IL-33) in serum were detected by enzyme-linked immunosorbent assay. CD4+T cells were isolated from the spleens of MRL/lpr and C57BL/6J mice at the age of 16 to 18 weeks by immunomagnetic beads method. The expressions of miR-200a and miR-155 and the levels of zinc-finger-enhancer binding protein 1(ZEB1) and suppressor of cytokine signaling1(SOCS1) in CD4+T cells were observed to explore the effect of AFPV on SLE and its possible mechanism. Result: Compared with the normal group, the diameter of back skin swelling in the model group was significantly increased (PPPPPPP+T cells of MRL/lpr lupus mice. Compared with the model group, the expression of microRNA-200a increased significantly, the expression of microRNA-155 decreased significantly (PPConclusion: AFPV has therapeutic effect on rats with SLE, its mechanism may be related to the regulation of miR-200a/ZEB1 and miR-155/SOCS1.

ABSTRACT

Objective:To explore the correlation between colors and contents of water and 5-hydroxymethylfurfural (5-HMF) in Ginseng Radix et Rhizoma Rubra (GRRR). Method:The colors were observed under sunlight on a sheet of white paper. The chroma value and color difference were determined and calculated by a colorimeter. Water content was determined by the oven-drying method. An HPLC method was established and used to determine the content of 5-HMF. The correlation was analyzed by the Spearman and Pearson analysis. Result:The colors of GRRR were yellowish-white,ocher-yellow,and yellowish-brown. The values of L*,a*,b*,and ΔE* had a certain range, and could reflect the colors of GRRR. The content of water ranged from 4.01%to 8.31%and was in accordance with the requirement the Chinese Pharmacopoeia. The content of 5-HMF ranged from below the limit of detection (1.985×10-4 μg) to 78.97 μg·g-1. The results of correlation analysis shows significant correlations between L*,a*,b*, ΔE* values and water content, that is to say,the deeper the color was, the higher the content of water was,but with no significant correlation with 5-HMF content. Conclusion:The color of processed GRRR was related to water content but not related to 5-HMF content,which was not consistent with the correlation between color and 5-HMF content that generally existed in a series of raw Chinese medicinal materials. It indicated an essential difference between the color shade of processed Chinese medicinal materials and the cause of color change of raw Chinese medicinal materials. Therefore,it was significant to control water content for ensuring the stability of GRRR. This study determined the colors of GRRR by a colorimeter, systematically determined 5-HMF content,and proposed the effect of water contents on the color of GRRR.

ABSTRACT

Objective: To investigate the current harvest,processing,packaging and storage of Lilii Bulbus due to the great difference in quality of commercially available products,in order to realize the factors affecting the quality after harvest and provide reference for the standardized production. Method: A series of factors affecting Lilii Bulbus quality were analyzed comprehensively on the basis of both field survey in main producing areas and medicinal material markets and literature retrieval. Result: The current situation and problems in harvest,initial processing,drying technologies,packaging,and storage were summarized. Conclusion: The harvest link of Lilii Bulbus involves growing years,harvesting time,and collecting methods. The initial processing link involves stripping,cleaning,sulfur fumigation,and many cooking and drying methods. The hot-air,vacuum-freeze,far-infrared,and microwave methods are applied in drying Lilii Bulbus. The main metamorphic phenomena are brown staining,moth biting,and decay in the packaging and storage links. Lilii Bulbus are packed mainly in bulk,and a few of them are packed in plastic bags or vacuum bags for fresh bulbus. Because the operation process is not standard,the cooking and drying methods are the key factors affecting the quality of Lilli Bulbus. It is necessary to establish a scientific and unified standard operating procedure (SOP) for controlling the whole process of Lilii Bulbus in multiple links.

ABSTRACT

Some samples of Asparagi Radix were collected from medical markets.Colors of Asparagi Radix were observed by human vision and recorded to judge whether samples were degenerative.Water content of Asparagi Radix was determined by a drying method.The chroma value and color difference were determined and calculated by a colorimeter.With the deepening of color,the L*value was decreased and a*and ΔE*values were increased.It showed that the results determined by colorimeter can replace the results of visual observation.An HPLC method was established and used to determine the contents of 5-hydroxymethylfurfural(5-HMF) in Asparagi Radix.The results showed the 5-HMF contents were from 0.002 255 to 0.049 14 mg·g-1 in some samples with yellowish-white or yellowish-brown color,significantly increased from 0.080 80 to 0.105 1 mg·g-1 in some samples with brown color,and up to 1.033 mg·g-1 in an oil-spilling sample with dark brown color.This result demonstrated that the 5-HMF contents were significantly increased by accompanied with the deepening of color.There were the significant negatively correlation between the 5-HMF content and the L*value(P<0.01) and positively correlation between the 5-HMF content and the a*or ΔE*value(P<0.01) by the spearman analysis.The oil-spilling and qualified samples were clustered into two alone categories by the cluster analysis.That the limited standards of the 5-HMF content is not higher than 0.02% by HPLC method and of the L*value is not less than 50 by colorimeter method were suggested for Asparagi Radix.It is firstly reported the multiple-factor analysis about oil-spilling and discoloration and the establishment of limited standard of Asparagi Radix.

ABSTRACT

Nutrition management based on enhanced recovery after surgery(ERAS) programs is a continuously evolving field in perioperative critical illness. Preoperative and post-operative nutritional measurement of ERAS are the core of perioperative nutrition. For critically ill patients, enteral nutrition assessment checklist may be able to improve the quality of enteral nutrition implementation. The management of gastric tube jejunal nutrition tube needs to be personalized according to the patient's condition. Auscultation-assisted bare-handed jejunal tube placement may make it easier for ICU patients to establish jejunal nutrition pathways.This paper will discuss nutrition management based on ERAS.

The role of inflammation in the pathomechanism and treatment of mood disorders has drawn public attention in the field.As representatives of central and peripheral immune systems,microglia and peripheral monocytic cells become more active and release relative cytokines in mood disorders patients.These two classes of immune cells have similar genetic expression and functions.It is beneficial to explore how microglia and monocytes influence emotion and behavior via immunc signaling pathway for understanding the disease pathogenesis and developing potential therapeutic strategies.

ABSTRACT

The efficacy and safety of normal saline (NS) for fluid therapy in critically ill patients remain controversy. In this review, we summarized the evidence of randomized controlled trials (RCTs) which compared NS with other solutions in critically ill patients. The results showed that when compared with 6% hydroxyethyl starch (HES), NS may reduce the onset of acute kidney injury (AKI). However, there is no significant different in mortality and incidence of AKI when compared with 10% HES, albumin and buffered crystalloid solution. Therefore, it is important to prescribe intravenous fluid for patients according to their individual condition.

Objective@#To investigate the expression characteristic of the Daxx gene in the mouse testis and its role in spermatogenesis.@*METHODS@#Real-time PCR, Western blot and immunofluorescence were used in examining the expression characteristics of DAXX in the testis tissue from wild-type, Sertoli cell-specific androgen receptor knockout (SCARKO) and androgen receptor knockout (ARKO) mice at different postnatal weeks .@*RESULTS@#The Daxx gene was highly expressed in the testis tissue and mainly in the nuclei of the wild-type mice at 4 postnatal weeks. Compared with the wild-type, the ARKO mice showed a markedly decreased expression of DAXX (0.299±0.026), which displayed a polar distribution in the spermatogenic cells (0.853±0.058) and exhibited no significant difference in the SCARKO mice (1.000±0.015).@*CONCLUSIONS@#The Daxx gene expression is the highest in the middle-stage development of the mouse testis, significantly decreased in ARKO mice as compared with the wild-type, and its location influenced by specific AR knockout in Sertoli cells. DAXX may be involved in the regulation of spermatogenesis in mice.

Objective@#To investigate the characteristics of the expression of the RIKEN cDNA 1700008O03 (1700008O03Rik) gene in the testis of the mouse from birth to sexual maturity and its potential role in regulating spermatogenesis.@*METHODS@#Using mouse gene expression profile microarray, we screened the testis-specific gene 1700008O03Rik from the mouse. We studied the expression characteristics of the gene in the development of the mouse testis by reverse transcription PCR, quantitative real-time PCR, Western-blot, immunohistochemistry and immunofluorescence, and analyzed the structure of the 1700008O03Rik protein and its homology with other species using the bioinformatic software.@*RESULTS@#1700008O03Rik gene was highly expressed in the testis of the mouse, increasing in an age-dependent manner, and mainly in the endochylema of oblong spermatozoa. Bioinformatic analysis revealed a high homology of the 1700008O03Rik protein between human and mice, and phylogenetic tree analysis showed it to be highly conserved in mammalian evolution.@*CONCLUSIONS@#1700008O03Rik is a highly expressed gene in the mouse testis, mainly in the endochylema of oblong spermatozoa, which may be involved in the regulation of spermatogenesis in mice.

ABSTRACT

Sepsis is a serious problem among the elderly population as its incidence and mortality rates dramatically increase with advanced age. More importantly, the elderly has increased vulnerability to developing sepsis due to diminished physiologic reserve, presence of comorbidities, immunosenescence and frequent instrumentation. Those who survive severe sepsis are more likely to have irreversible organ damage, cognitive impairments, and diminished overall function. Additionally, elderly patients with sepsis often present with atypical symptoms which further complicates and potentially delays diagnosis. Although sepsis is a serious life-threatening disease, recognition of this problem is very low compared to other age-associated diseases. Therefore, the purpose of this review is to analyze the challenges facing this cohort and how to optimize their management.

ABSTRACT

Objective To detect the hepatotoxicity biomarkers using normal human hepatocyte (HepaRG) and high-content screening,and to combine the micronucleus test and single cell gel electrophoresis to estalish a rapid screening platform for in vitro cytotoxitity and genotoxicity.Methods The effects of rhubarb anthraquinones (AQs) on the reactive oxygen species (ROS),intracellular Ca2+ concentration and mitochondrial membrane potential (MMP) in HepaRG cells were studied using appropriate fluorescent probes Hoechst33342、DCFH-DA、Fluo4-AM、Mito Tracker Red CMX Ros and high-content screening methods,and the potential genotoxiciy triggered by AQs were analyzed using the high-content based cytokinesis block micronucleus test and high throughput comet assay.Results The intracellular ROS level of HepaRG cells was significantly elevated by a 24 h treatment with Emodin (25.0 μg/mL),aloe-emodin (25.0 μg/mL) or chrysophanol (50.0 μg/mL),which are dose-concentration dependent (P < 0.05 and 0.01);the intracellular Ca2+ increased and mitochondrial damage were observed with the treatment of aloe-emodin (25.0 μg/mL) and rhein (50.0 μg/mL,P < 0.05 and 0.01).Comparing to control group,Emodin (25.0 μg/mL) induced an increased micronucleus rate (1.59% ± 0.68 %,P < 0.01) and significantly higher percentage tail DNA and Olive tail moment (respectively 10.155% ± 2.17% and 0.510 ± 0.06,P < 0.05 and 0.01) after 24 h;while the chrysophanol increased the micronucleus rate to 1.29% ± 0.54% (P < 0.01) after 72 h.Conelnsion The results on the cytotoxicities and genotoxicities of AQs are consistent with the literatures.In this study,a rapid screening model for both hepatotoxicity and genotoxicity was successfully established,which will help with the early screening during the drug development stage.

Objective To study the effect of supermicro needle-knife plus rehabilitation on the strephenopodia angle and lower-limb motor function in cerebral stroke patients.Method Sixty cerebral stroke patients were randomized into a treatment group and a control group by using random number method, 30 cases each. The control group was intervened by rehabilitation training, while the treatment group was additionally given supermicro needle-knife therapy, for successive 4 weeks. The strephenopodia angle and Fugl-Meyer Assessment of lower-limb motor function (FMA-L) were observed before and after the treatment.Result The strephenopodia angles and FMA-L scores were changed significantly in both groups after the treatment (P<0.05); after the treatment, the strephonopodia angle and FMA-L score in the treatment group were superior to those in the control group, and the differences were statistically significant (P<0.05).Conclusion Supermicro needle-knife plus rehabilitation can mitigate strephenopodia and improve lower-limb motor function in cerebral stroke patients.
